I am a highly motivated HCPC registered physiotherapist with an accredited Bachelor’s degree in physiotherapy. I am a member of the Nigeria Society of Physiotherapy (NSP); Association of Clinical and Academic Physiotherapists of Nigeria (ACAPN) both registered with Medical Rehabilitation Therapists Board of Nigeria and I intend to join the CSP as soon as possible. I am a passionate, goal driven physiotherapist committed to delivering high quality patient-centered care in accordance with the Health and Care Professional Council (HCPC) and Chartered society of physiotherapy (CSP) professions ethics and standard of practice. I have had over 10 years post-graduation experience in Nigeria which has afforded me the opportunity to treat a wide range of patients from acute to rehabilitative phase in a multidisciplinary setting where goal oriented programs were designed in collaboration with the patient. As a Physiotherapist at the state specialist hospital Akure now referred to as University of Medical Science Teaching Hospital Akure Nigeria, I had a broad rotational experience through the major clinical postings including Neurology/Stroke, Rehabilitation, Musculoskeletal Outpatients, Orthopaedics, Acute Respiratory Care (Medical/Surgical/Critical Care), Paediatrics, Integrated Discharge Team and Community where I managed varieties of conditions such as stroke, dementia, Parkinson disease, COPD, peripheral neuropathies, multiple sclerosis, low back pain, joint pains, post-op hip and knee replacement, knee osteoarthritis, motor neuron disease etc. As a band 6 physiotherapist, I managed my clinical caseload within a defined clinical area working autonomously, but with access to advice and support from Chief physiotherapy staff while providing physiotherapy assessments over a range of conditions, using clinical reasoning skills and techniques to formulate accurate physiotherapy diagnoses, deliver evidence based physiotherapy treatment programs, to meet the agreed goals set with the individual patient and/or carer; also evaluate patient progress, revisit goal setting and alter treatment programmes accordingly. During this period, I gained experience in writing up patient notes accurately and independently, understanding and interpreting signs and symptoms effectively and, I became confident in overcoming barriers to appropriate rehabilitation, including communication difficulties, anxiety or motivation as I use a range of verbal, non-verbal and de-escalation and conflict resolving skills to communicate effectively with patients to progress rehabilitation and treatment programmes; these includes using sign to pass messages across. My rotations has afforded me the opportunity to work in alliance with members of the multidisciplinary team involving neurologists, orthopaedic consultants, speech therapists, occupational therapists, psychologists, dietitians, social worker and also patients caregivers and relatives etc. involved in planning and implementing care. This approach to patient management has encouraged my team work spirit by enhancing multi-disciplinary and holistic approach to patient care and build my relationship skills with them all. As part of an integrated health and social care team, I support the multidisciplinary team in providing care to avoid inappropriate admissions to hospital and support hospital discharge. Currently, I am a band 7 physiotherapist with keen interest in community rehabilitation, I am more actively involved in home visits and assessments of patients who are at risk of hospital admission or readmission, being a car driver with a full driving license makes travelling easy. I maintain a high standard of clinical care for patients under my management and I complete advanced risk assessments independently within the patients own home, formulate and deliver individualized physiotherapy management and treatment programme, based on evidence based practice and expert clinical knowledge.
